The concerned CGHS offices supply
to various local Central Govt. offices etc. blank Index as
well as Identity Cards bearing Identical Serial Numbers.
The issuing authority gets the Index Card & Identity
Card filled from the intending beneficiary in duplicate giving
the names of the beneficiary & members of his family eligible
for CGHS facilities, showing their dates of birth, relationship
with the Government Servant, etc.
One copy of the Index Card/Identity Card is retained by the
issuing CGSH office. Second part of the Index Card/Identity
Card shall be forwarded to the Dy. Director / Chief Medical
Officer of CGHS of the respective cities. The Third part shall
be handed over to the employee concerned.
An attested photograph of the family members of the beneficiary
shall be affixed on the Identity Card duly signed by the officer
issuing the card.
The employee shall deposit the Index Card with the Medical
Officer In charge of the allotted dispensary and retain the
Identity Card with himself. The fact of submitting the Index
Card should be acknowledge or endorsed on the identity Card.
For availing medical facilities, the beneficiary must show
his Identity Card on every visit to the Medical Officer posted
at the dispensary.
Thereafter, the concerned Govt. employee & members of
his family become entitled to receive CGHS facilities in the
allotted dispensary.
On transfer from one dispensary to another, the card holder
must collect the Index Card from the previous dispensary and
deposit it at the new dispensary along with transfer application
form (in duplicate)duly signed by the Issuing Authority of
his office/department.
In the case of pensioner, the card holder should collect
the index card from the dispensary and deposit it with the
new dispensary alongwith the transfer form duly signed by
the Medical Officer. The signature of the issuing Authority
is not require in this case.
Any change in the Residential address has to be reported
by the beneficiaries to the Identity Card issuing Authority
for necessary amendment in the Index Card.
In case of transfer of beneficiary to another office, the
Index Card is transferred to the new office of the beneficiary.
In case of transfer of beneficiary to another office, the

In the case of change of residence necessitating change of
dispensary, the beneficiary, on a formal application to the
issuing authority is given his Index Card, which he gives
to his previous dispensary. The medical officer Incharge of
the dispensary, after making the necessary changes in the
Index Card gives the same back to the beneficiary who them
deposits the same with the new dispensary allotted to him.
2. Affixing of Photographs of CGHS Token Card of
Serving Employees and their Dependents
With effect from 1-1-98 the photography of their family and
dependents, if any, availing CGHS facilities should be affixed
on the CGSH cards issued to them. The infants / babies / children
up to the age of 5 years are exempt from the provision. The
